Community Health Worker Awareness Week is being celebrated this year from Aug. 24-28, and the Health Federation is proud to salute the Community Health Workers/Peers on the front lines in Philadelphia and beyond.

The Health Federation of Philadelphia was awarded a two-year, $400,000 grant from The Barra Foundation to test whether a proactive, technology-enabled system can help health center patients who are insured by Medicaid maintain health coverage amid new administrative barriers.

The annual celebration of National Health Center Week provides an opportunity to support and highlight the crucial and innovative work done all year long by health centers both in the Philadelphia region, as well as statewide and nationwide.
