The Health Federation of Philadelphia’s Health Center Controlled Network brought together healthcare professionals from around the state for its two-day Learning and Networking Workshop early last month.
A total of 55 attendees were on hand from 19 Federally Qualified Health Centers statewide for the event, held at Normandy Farm Hotel & Conference Center in Blue Bell, Pa.
The total number of attendees from health centers was a record for the event.
“The annual HCCN workshop is always a great opportunity for participating Pennsylvania health centers to learn from each other, from the HCCN team, and from outside experts about FQHC innovation and challenges in health information technology and population health,” said Suzanne Cohen, Senior Director of Population Health at HFP.
The first day of the workshop began with a report out and kickoff from the HCCN team’s Holly Nagy Hainsworth, Director of Population Health Initiatives at the Health Federation.
Next were highly engaging presentations and breakouts on Operationalizing AI: From Implementation to Impact, facilitated by Khai Williams, Director of Clinical Informatics at the Health Federation. Panelists included Sue Carter (Family Practice & Counseling Services Network), Michael Colli (Keystone Health), Joshua Warfel (River Valley Health), and Eric Rios (Spectrum Health Services).
Williams next facilitated the Data-Driven EHR (Electronic Health Record) Transitions presentations and Q&A session, which were followed by EHR roundtables over lunch. Panelists included Isaiah Adio and Karen Wang (Berks Community Health Center), Kristie Lenze (Keystone Rural Health Consortia), Maria Hall and Howard Kahlenberg (Project HOME), and Kara Poremba (Wayne Memorial Community Health Centers).

The afternoon of the first day brought QI/PHM Panel: Inside High Performing Health Centers presentations and breakout groups, facilitated by Jessica Chen, Director of Quality Improvement & Evaluation at the Health Federation. Panelists included Candace Neiman (Community Health & Dental Care) and Theresa Moran and Ryan Obsniuk (Spectrum Health Services). A Privacy and Cybersecurity Tabletop Exercise capped off the first day of the workshop.
Day 2 of the workshop was focused on Azara DRVS, a data reporting and analytics tool used by most HCCN member health centers, and featured 3 hours of hands-on training facilitated by Azara’s Clinical Support Team. Azara also had a member of their Support Team on hand throughout this year’s 2-day session to provide additional troubleshooting and support for participants.
Led by Holly Nagy Hainsworth, the Director of Population Health Initiatives at the Health Federation, the HCCN team includes: Suzanne Cohen, Senior Director of Population Health; Jessica Chen, Director of Quality Improvement and Evaluation; Khai Williams, Director of Clinical Informatics; Emma Moulder, Program Manager; and Haley Slezosky, Quality Improvement Specialist.

The HCCN is a grant-supported program open to Federally Qualified Health Centers and look-alikes across Pennsylvania. The project focuses on supporting patient-centered health care delivery and population health. The HCCN helps health centers to optimize use of health information technology and population health management tools and supports quality improvement efforts such as data collection, management, analysis, and reporting. The HCCN data warehouse represents de-identified health services and outcomes data for nearly 400,000 health center patients in Pennsylvania.
