Community Health Worker Awareness Week is being celebrated this year from Aug. 24-28, and the Health Federation is proud to salute the Community Health Workers/Peers on the front lines in Philadelphia and beyond.

Community Health Workers (CHWs) can be found across health and social service organizations, lifting people up in times of need and helping residents navigate complex systems and life circumstances.

"This Community Health Worker Awareness Week, the Health Federation of Philadelphia is thrilled to shout out the amazing community of Community Health Workers/Peers we are privileged to work with and serve every day from across the Philadelphia region," said Kiera Kenney, program director. 

"Today we send a special thanks to our team of CHWs at the Health Federation: Tamara M. Cobb, Maggie Green, Julia Galante, Bryana Fisher, Austin Zeng, and every CHW who has shared their wisdom, empathy, skills, and passion with our neighbors," Kenney said. "We value you!"

The Health Federation's Peer Models for Health Learning Collaborative supports a growing community of clinical and community-based Community Health Workers, Peers, and Supervisors dedicated to addressing social drivers of health and reducing health disparities in North Philadelphia's Health Enterprise Zone and across the city.

Through monthly facilitated conversations, trainings, practice sessions, consultations, and networking opportunities, the Learning Collaborative brings together a community of passionate people with varied experiences, skills, and strengths who share a commitment to continuous development and mutual support.
