What is your role and what does your average day look like?

I am a National Health Corps member that volunteers with the care coordination team at Fairmount Primary Care Center. An average day for me consists of outreaching to the health center’s patients to complete a social determinants of health screening. This screening gives the health center more insights into the needs of their patient population, and it gives me the chance to talk with patients about relevant resources they may be interested in utilizing. 

What is your favorite part about your role?

My favorite part is being able to serve the patients of Fairmount Primary Care and help them address the social factors that may contribute to their health outcomes. By working closely with patients, I have gained a deeper understanding of the community and its needs. Additionally, earning a patient’s trust and the confidence to share with me their stories and the challenges they face has had a lasting impact on me. 

What were your initial impressions of the platform?

Because I am not originally from Philadelphia, exploring Connect4Health was a great way for me to familiarize myself with what the city had to offer, in terms of government, philanthropic, and other useful resources available to the city’s residents. I was most excited to use Connect4Health because of its ability to narrow the search for resources based on the patient’s specific demographics. This feature would be useful for individualizing the resources best suited for the patient’s needs.

How are you currently using the platform? How has Connect4Health supported your work?

When a patient expresses a need relating to social factors, such as food, or utilities, or transportation, I mainly use the Connect4Health platform to research what resources are available for them to connect with. The platform has been an integral part of my work to help patients access beneficial resources.
