JOB SUMMARY

This position works with the Director of Quality Improvement and Evaluation to support performance measurement, quality improvement, and evaluation activities within the Population Health department at the Health Federation of Philadelphia (HFP).  The Population Health department uses a population health management system, Azara DRVS, to aggregate data from participating health centers and support quality improvement and evaluation activities at the health center level and across several projects.  The QI Specialist will support programs within the department, including the Health Center Controlled Network (HCCN), which serves 35 participating health centers across the state of Pennsylvania, and the CDC Diabetes 2320 program, which aims to prevent or delay the onset of type 2 diabetes in adults with prediabetes and to enhance quality of care among people with diabetes.   Work on additional initiatives may include supporting health centers in reviewing and utilizing data to improve cardiovascular health, cancer screening and follow-up, and engagement in value-based care contracts.

JOB SPECIFICATIONS

Responsibilities/Duties

  • Engage in quality improvement and evaluation activities with health centers and Population Health department programs at HFP.  This will include assisting with needs assessments, researching quality improvement strategies and tools, and assisting with evaluation tool design and administration.
  • Develop reports and dashboards in the population health management system (Azara DRVS) to support QI projects related to topics such as hypertension control, diabetes care, cancer screening and follow-up, and value-based care contracts.
  • Support efforts to assure data quality in the population health management system, including data mapping support and other data validation tasks.
  • Assist in providing training and technical assistance to health center partner organizations in use of the population health management system. 
  • Support preparation and presentation of QI data reports to health centers, funders, and other partner organizations.
  • Utilize system data and tools to generate reports and dashboards in support of quality improvement and evaluation activities. 
  • Research QI change ideas and QI tools.  Provide support in curating and presenting such ideas and tools to project teams and QI work groups.
  • Assist in facilitating QI work group and other network-level meetings focused on data and quality improvement (support taking attendance, meeting minutes, and identifying follow-up tasks).
  • Support development and implementation of project evaluations within the Population Health department (including evaluation planning, tool development, data collection/management).

Education:  Bachelor’s degree in public health, social work, sociology, health information technology or related field; 2-4 years of relevant experience; or a combination of education and experience that reflects the skills identified in this position description.

Skills/Experience:

  • Experience with data management, analysis, interpretation, and visualization.
  • Proficient in Microsoft Excel, Word, and Power Point. Ability to quickly master new technology tools is essential.
  • Strong written and verbal communication skills.
  • Experience working on program evaluations and/or projects to support data-informed quality improvement preferred.
  • Highly organized, with excellent time management skills, and systems-oriented.
  • Strong problem-solving and collaborative skills.
  • Demonstrated ability to work independently and as part of a team.
  • Able to handle multiple tasks and manage project timelines with a high degree of organizational skill.

Work Environment:  Standard office setting with extended periods at work station and periodic use of office equipment.

Position Type and Work Schedule:  Full time position, typical hours are Monday through Friday 8:30am to 5:00 pm.  Flex office schedule options available with supervisor approval.

Travel:  Minimal scheduled travel for meetings, trainings, and technical assistance sessions with participating health centers.

Physical Demands:  Able to sit and work at a computer keyboard for extended periods of time.  Able to lift and move up to 15 pounds.
