BACKGROUND
National Health Corps: Community Health Fellowship Greater Philadelphia is now recruiting members for their upcoming cohort!

The Community Health Fellowship program recruits, trains, and places 40 individuals in resource-limited organizations in their communities across the country.  All NHC Community Health Fellowship, Greater Philadelphia members must be residents of host site communities. Most members complete 1,700 hours of service during a 42-46 week service term.  These NHC: CHF members help build healthy communities by providing essential health education and services to individuals in communities plagued by histories of racial and economic inequities, while developing tomorrow’s compassionate health leaders.  NHC welcomes people with all different interests and future goals!  The most important quality in an NHC member is a readiness to bring the best they have to offer - their creativity, energy, enthusiasm, and readiness to learn - to serving their communities. 

All NHC: CHF Greater Philadelphia members must live, work, or go to school in the Greater Philadelphia communities of Philadelphia County, Delaware County, Chester County, Montgomery County, Bucks County, or Camden County (NJ).

POSITION INFORMATION
Program Name:               National Health Corps: Community Health Fellowship Greater Philadelphia
Member Position:           Patient Navigator
Host Site:                           Spectrum Health Services 5201 Haverford Avenue, Philadelphia PA 19139

Position Summary: Spectrum Health Services is a Federally Qualified Health Center that provides integrated care services for patients facing the complexity of medical and social/economic issues that emerge across the life cycle.

The Patient Navigator will play an important role at Spectrum Health Services' Community Health Center clinic located in West Philadelphia and provide supplemental support to our Cottman Avenue Site. The member will serve as an intermediary between patients and their families and the larger Spectrum care team, fostering trusting relationships and connecting patients to the range of health and social services available to address their needs. The member will help patients navigate Spectrum and specialty health system resources, connect with community programs, sign up for benefits, assist with form-related questions and completion, and work with patients to adopt healthy behaviors.

Major Duties and Responsibilities

  • Completing and submitting patient applications for social services and medication assistance programs
  • Providing education on how to complete paperwork
  • Conducting phone outreach as assigned by leadership
  • Assisting in care coordination and resource referrals
  • Creating social service resource guides and lists for clients

  • Demonstrated interest in Social work, health and wellness
  • Hands on assistance with getting patients connected to external specialty care and social resources to which they were referred

Position Type and Work Schedule:
This is a full-time, non-exempt position.  Members complete 1,700 service hours over a 42-46 week service term.

Work Environment:
Standard office setting, plus minimal scheduled travel for meetings, trainings, and service opportunities.

Physical Demands:
Able to sit and work at a computer keyboard for extended periods of time.  Able to lift and move up to 15 pounds.

Travel:
Must be able to get around effectively within Philadelphia and surrounding counties, either on public transportation and/or in own vehicle.
