JOB SUMMARY

 The Health Federation seeks a qualified and detail-oriented Community Partnership Coordinator to serve in this full-time position.

The Community Partnership Coordinator will be working with the Integrated Peer Models for Health (IPMH), a program funded by the Pennsylvania Department of Human Services. This program supports capacity building within the North Philadelphia Health Enterprise Zone (HEZ) to benefit populations facing multiple barriers to health and wellbeing, including environmental and mental health/substance abuse challenges, social determinants of health, and chronic disease. 

The Community Partnership Coordinator will play an important role in the IPMH team and work with three pillars of the program, Connect4health, Learning Collaboratives, and the Diabetes Prevention Program. See below the responsibilities and duties of each pillar. To be successful this person must be a lifelong learner, able to lead and work collaboratively, self-starter, comfortable assessing priorities, and adept at using new technologies/solutions. 

JOB SPECIFICATIONS

Responsibilities/Duties

Connect4Health (50%) 

  • Work with the IPMH Assistant Director of Programs to identify and make meaningful connections to valued programs and services focused on our partners’ needs 
  • Utilize platform data to support CBO engagement strategy planning and community outreach efforts
  • Increase the visibility and effective use of the findhelp/Connect4health platform across local CBOs focused on the North Philadelphia Health Enterprise Zone
  • Attend relevant training webinars and opportunities to enhance learning and apply learning where possible
  • Support the continued optimization of platform resource listings and user navigation through convening the Connect4health advisory group feedback quarterly meetings
  • Create and distribute periodic newsletters to Connect4health partner organizations
  • Contribute to program monitoring, evaluation, and improvement activities
  • Represent the Health Federation of Philadelphia in internal and external meetings/calls and assist with documentation of minutes and follow ups

Learning Collaboratives (30%)

  • Create and send out invitations, evaluations, and participation certificates, as well as timely emails to participant list ahead of and following learning collaborative sessions. Manage registration
  • Coordinate with PA Certification Board to submit complete session content for approving continuing education hours where available
  • Ensure participant list is kept up to date 
  • Interview new learning collaborative members and coordinate work with the Peer Models for Health Advisory Board
  • Provide support for and participate in learning collaborative sessions

Diabetes Prevention Program (20%) 

  • Support and monitor the CDC data collection and reporting processes
  • Provide patient recruitment support 
  • Plan and monitor the purchase of incentives for participants of the program
  • Contribute to the development of a strategy to engage participants throughout the 1-year program

Education

  • Undergraduate degree in Social Work, Public Health or other relevant study areas preferred.
  • Degree not required if experience and skills requirements are met. 

Skills/Experience

  • 3+ years experience doing community engagement work and data support, 
  • Ability to quickly learn a new platform and be able to demonstrate its use
  • Excellent written and verbal communication skills 
  • Thorough attention to detail and organization skills
  • Outstanding interpersonal skills, with a background in interacting with culturally and racially diverse people and organizations
  • Prior experience with community engagement and customer service a plus
  • Basic understanding of the value and challenges of small and large CBOS a plus
  • Comfortable using Microsoft Suite, Google (e-mail, calendar, Drive), and other platforms, as needed
  • Demonstrated ability to work independently.

Work Environment:

The Community Partnership Coordinator will work in a 60/40 hybrid format (60% of the time in-office and 40% of the time virtual) and will report to the Health Federation of Philadelphia, 123 S. Broad Street, Suite 650, Philadelphia, PA 19109. Various meetings and training will be scheduled off-site throughout the city of Philadelphia. 

Position Type and Work Schedule:  Full Time, 37.5 hours per week (5 days/week) 

Travel:  Semi-frequent local travel to meetings will be expected.

Physical Demands: This position requires sitting at a computer and desk, traveling to organizations throughout the city and carrying supplies and equipment weighing up to 20 pounds.
