JOB SUMMARY
The Community Health Worker will identify high-risk individuals and link them to care. They will be responsible for advocating for expanding access to preventative services and treatment through internal and external community engagement, health education, and intra-agency collaborations.
Primary Duties and Responsibilities:
- Conduct programmatic activities working closely with the HIV/AIDS Priority Clinic team members;
- Establish rapports with community agencies to identify individuals at high risk for HIV and link them to care within AHS;
- Liaise with agencies serving individuals at high risk for HIV to provide education on HIV prevention and treatment services offered at AHS;
- Act as a main point of contact for referrals;
- Maintain accurate documentation of referrals and outreach attempts;
- Coordinate with Priority Team to facilitate and integrate patients into care;
- Participate in community health events to strengthen partnerships and community engagement;
- Develop resource guides to enhance workflows and ensure proper route of referrals;
- Address barriers to preventative services, testing, and treatment;
- Gain and utilize knowledge of the city of Philadelphia’s health center system and additional HIV resources in Philadelphia;
- Comply with all program protocols;
- Attend all HIV Priority Team meetings, site visits with funders, and required trainings;
- Adhere to the HIV program harm reduction approach (meeting people where they are and withholding judgment about choices they make, etc.);
- Provide services in accordance with the standards set by Ambulatory Health Services, DHH, and Center for Disease Control;
- Perform other duties as assigned.

Education Required: High school diploma or higher
Work Environment: Standard office setting with extended periods at workstation and periodic use of office equipment. Some community-based work required.
Position Type and Work Schedule: Part time position: typical hours are Monday through Friday 8:30 am to 5:00 pm, days worked TBD. Flex office schedule options available with supervisor approval.
Travel: Local travel to multiple sites and community meetings required.
Physical Demands: Ability to transport materials and move equipment or supplies weighing up to 30 lbs. on a limited basis.
