JOB SUMMARY

The Certified Registered Nurse Practitioner (CRNP) will be a significant part of Philadelphia’s Department of Public Health (PDPH) Ambulatory Health Services (AHS) Priority Clinic. This position is advanced level nursing work, providing comprehensive HIV treatment and preventative care.

The CRNP will have internal contacts with the entire administrative staff and external contact with patients, community resources, and the community. This position has access to sensitive information and is expected to handle such information with integrity and professionalism. This position has regular contact with members of the community and is expected to represent the City of Philadelphia in a professional manner.  The CRNP will be based in the City of Philadelphia Health Centers and in the communities surrounding the City of Philadelphia Health Centers. 

The CRNP will report directly to an overseeing supervisor and participate in Department activities as necessary, as well as any trainings or direction set forth by the health center. 

JOB SPECIFICATIONS

               Responsibilities/Duties

  • Conducts baseline and ongoing patient assessments for specified program participants
  • Diagnoses medical conditions of patients under the supervision of a medical superior
  • Develops treatment plans and behavior modification plans for health maintenance
  • Orders appropriate laboratory tests and x-rays and refers patients with complications
  • Prepares reports of findings and re-examines patients for recurrence of poor health outcomes
  • Educates patients about health issues to improve community health and safety and increase access to care
  • Monitor health trends and identify health risk factors unique to specific communities
  • Tell Philadelphia residents about locally available health care programs and services to improve access to care
  • Work with patients to identify barriers to care and implement interventions with the assistance of the health center staff to remove barriers to care
  • Provide direct health care services, including preventive care, screening services and health education to vulnerable and at-risk populations
  • Conduct risk reduction counseling and partner counseling and provide referral services with all patients to prevent HIV transmission
  • Attend relevant workshops or join professional groups as necessary to maintain professional knowledge and licensure.
  • Attend relevant staff meetings to promote communication and execution of goals
  • Complete special projects specific to the function of the department or as needed for the department as directed by Supervisor
  • Other duties as assigned within the scope of position expectations

Education

  • Minimum Education: Masters degree required
  • Certification/License: Possession of Licensure as a Certified Registered Nurse Practitioner as issued by the Pennsylvania State Board of Nursing Education

Skills/Experience

  • Minimum Experience: 2+ years experience providing direct services to a diverse population in a human services, health care, community or educational setting
  • Experience with populations at risk for HIV is preferred 
  • Bi-lingual in English/Spanish or English/French is also preferred
  • Principles, practices and current developments in the area of specialization
  • Principles, practices and current developments of general nursing
  • Principles and practices of preventive medicine
  • Medical jurisprudence
  • At least one year of experience working in the field of HIV
  • Basic understanding of the City of Philadelphia’s mission, vision, values, programs and services and business plan
  • Ability to maintain appropriate and professional judgment in interactions with patients and staff
  • Ability to maintain a respectful, nonjudgmental, and compassionate manner with patients
  • Knowledge of community resources and benefits that impact and/or benefit the targeted patient needs
  • Ability to effectively use standard office equipment
  • Possess strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, external business partners, and the community
  • Ability to operate a computer and use a variety of common software programs including Microsoft Office, spreadsheets, and customized databases
  • Strong written and verbal communication skills and effective communication with individuals and groups
  • Ability to work in a team structure – demonstrating ability to collaborate and contribute to the team’s work
  • Adherence to all City of Philadelphia and departmental policies and procedures
  • Attendance at all City of Philadelphia in-services as required

Work Environment:  Standard office setting with extended periods at work station and periodic use of office equipment. Some community based work required.

Position Type and Work Schedule:  Part time position; typical hours are Monday through Friday 8:30 am to 5:00 pm, days worked TBD.  Flex office schedule options available with supervisor approval. 

Travel:  Local travel to multiple sites and community meetings required.

Physical Demands:  Ability to transport materials and move equipment or supplies weighing up to 30 lbs on a limited basis.
