About HCCN

HCCN is a grant-supported program open to Federally Qualified Health Centers and look-alikes across Pennsylvania. The project focuses on supporting health centers to leverage health IT to increase their participation in value-based care. The HCCN optimizes health center use of information technology and population health management tools and supports data standardization, management, and analysis, and value-based payment initiatives.  The HCCN data warehouse represents de-identified health services and outcomes data for over 500,000 health center patients in Pennsylvania.

Benefits of Participation

When you join the HCCN, you will receive technical assistance and support around population health management and data warehouse tools like i2i Tracks and PopIQ, and both individualized and network-level expert health IT consultation.  Expert and peer support groups, EMR-specific user groups, and additional opportunities for networking and collaboration across the Network provide additional assistance and support for HCCN participants. 

Project Examples

  • Consultation and TA on implementation and/or optimization of a patient portal or other patient engagement tools

  • Convening of Chief Information Officers/IT Directors network-wide

  • Implementation, optimization and data analysis support for PRAPARE and platforms supporting SDOH referrals

  • Data validation and reporting support, including support around standardizing documentation, data mapping, and report and dashboard building

  • Consultation around value-based payment activities

 

Your Role

Through calls and meetings, participants work with a project team to identify and address a work plan specific to your health center.
