Health Federation Awarded Grant by Avantor Sciences Foundation to Pilot a Digital Navigator Program

Health Federation was awarded a grant by the Avantor Sciences Foundation to support piloting a digital navigator program in community health centers.  As the healthcare system increasingly moves to include online patient portals, telehealth, and remote monitoring, low-income patients lacking access to high speed internet and devices can easily lag behind. Digital navigation helps patients gain access to and use technology to connect with essential health care services. Through this program, HFP will hire a digital navigator, develop training for this role, and deploy the navigator to one or more community health center sites in the Philadelphia area. There, the digital navigator will help patients to apply for government benefits that can support access to devices and high speed internet, and assist individuals in understanding and learning to use health-specific applications.
