JOB SPECIFICATIONS

Responsibilities/Duties

  • Conduct programmatic activities working closely with the HIV/AIDS Priority Clinic team members;
  • Comply with all program protocols;
  • Complete AHS orientation and all mandatory trainings of program to guarantee appropriate competency in program requirements;
  • Obtain verbal consent from patients for HIV testing or documentation of reason for refusal;
  • Provide one-on-one and group-centered education on HIV/AIDS, risk behaviors, substance use and harm reduction;
  • Administer rapid HIV test to patients who have consented and perform fingerstick confirmatory and baseline testing as indicated;
  • Provide HIV results and post-test counseling to patients;
  • Facilitate linkages to HIV social worker for HIV-positive and high risk HIV-negative patients, and linkages to HIV care centers as needed;
  • Facilitate linkages to care for HIV-positive patients referred from community-based organizations, or self-referred;
  • Facilitate linkages to PrEP services for high-risk patients testing negative;
  • Follow-up with patients, health center social workers or service providers and agencies to determine outcome of referrals;
  • Establish and maintain relationships with providers to ensure appropriate referrals and outreach;
  • Provide accurate information to the community about HIV transmission, prevention and testing;
  • Keep accurate and comprehensive charts on patients, including information on patient interviews, counseling sessions, referrals and HIV counseling and testing;
  • Perform accurate data entry using internal database or manually submit paperwork in a timely fashion;
  • Participate in HIV priority team, staff and program meetings, site visits with funders and required trainings;
  • Adhere to the HIV program harm reduction approach (meeting people where they are and withholding judgment about choices they make; etc.);
  • Provide services in accordance with the standards set by Ambulatory health Services;
  • Perform other duties as assigned

Qualifications

Education

High School Diploma or equivalent

Skills/Experience

Preferred skills

  • Ability to work fluidly to deal with different cultural norms, educational backgrounds, and professional skills;
  • Ability to perform tests involving bodily fluids;
  • Ability to engage staff and patients in no-condescending and empowering way;
  • Have strong conflict resolution skills;
  • Highly motivated, detail-oriented with good communication and computer skills
  • Excellent oral communication skills
  • Dynamic, creative, flexible, ability to learn and adapt

Work Environment:

Standard office setting with extended periods at workstation and periodic use of office equipment.

Position Type and Work Schedule:

Monday through Friday 8:30 am to 5:00 pm.
