JOB SUMMARY

The clinical care coordinator is an advanced practice nurse (nurse practitioner) who works autonomously or in collaboration with other healthcare professionals to deliver quality care.  The clinical care coordinator offers a wide range of healthcare services directed at the student and her/his family from the promotion of a healthy lifestyle to disease prevention, management of chronic health conditions, identification of an acute health need and health counseling.  The clinical care coordinator sees the student and parent(s)/guardian as partners in their care with an emphasis on health and well-being to maximize the student’s ability to learn and thrive.

JOB SPECIFICATIONS

  • Develops and monitors implementation of effective clinical practices in the delivery of student health support services
  • Serves as a resource to the school nurse; works collaboratively to manage complex health needs of students.  Supports the school nurse in public health outreach to families to ensure school readiness for students.  Serves as a primary clinical consultant to school nurses
  • Coordinates care with community resources and establishes a system to link students, parents, guardians and primary care providers
  • Establishes and maintains positive relationships with managed care companies as well as other public, private and governmental organizations.  Serves as a liaison with stakeholders invested in the health care needs of children
  • Ensures adherence with all state, federal and local mandated guidelines governing activities related to school-based health services
  • In collaboration with the school nurse, identifies the health status of students through a comprehensive health history, recommending appropriate diagnostic and screening tests as indicated
  • Provides one-time prescriptions of medications as indicated and communicates with the primary care provider regarding current health status and need for follow up care
  • In collaboration with the Director of Nursing and Medical Director, reviews current nursing standards of practice and revises or rewrites as necessary
  • Cooperates with community health organizations, in coordination with the school nurse, in ensuring that immunizations for students are current.  Assists in immunizations as needed
  • Serves as a consultant to school and central office staff and works collaboratively on student-related health needs and the planning, development and implementation of school-based health services
  • Identifies best practices for delivery of health services across the District and within non-public schools
  • Supports the implementation and maintenance of programs and practices that support the goals and objectives of the School Board for healthy children and families
  • Implements and evaluates the effectiveness of professional development activities for school nurses; provides training on current issues in school-based health care either directly or through trainers.  Coordinates monthly regional nursing meetings to advance professional development
  • Remains current in her/his practice and professional responsibilities, to include but be limited to chronic disease management and acute medical or mental health needs
  • Develops care plans and implements treatment plans based on clinical decisions
  • Identifies and participates in the development of grant proposal opportunities
  • Provides excellence in school health services in accord with established standards of care and practice by the Pennsylvania Board of Nursing as well as the credentialing bodies for school nurses and nurse practitioners and other state, local and federal regulations
  • Participates in school sponsored community health education and health promotion initiatives
  • Provides oversight of schools and school nurses in mandated service delivery, including the Commonwealth of PA screenings and examinations and the Philadelphia Department of Health immunization compliance regulations
  • Utilizes the Student Information System (SIS) and other data sources to evaluate the effectiveness of school health services
  • Maintains and reviews the School Nurse Procedures Manual and references it as a basis of support for schools and school nurses

Education/Certification

  • Graduate of a master’s degree program in nursing from an accredited university (MSN)
  • PA Board certification as a nurse practitioner (CRNP)Minimum of a High School or GED Diploma

Skills/Experience

  • Three years plus experience as a NP in pediatrics or family practice preferred
  • Current registered nursing license (RN) in PA in good standing
  • Excellent written and verbal communication skills
  • Awareness of standards of practice, NCQA guidelines and Medicaid contracts and benefit systems preferred
  • Eligibility for prescriptive authority through the Board of Nursing or Medical Board in PA required

Work Environment:

Public elementary school in Philadelphia

Position Type and Work Schedule:

Full time position, typical hours are Monday through Friday from  8:30 am to 5:00 pm.  Some evenings and weekend hours are likely.  Flex office schedule options available with supervisor approval. 

Travel:

Occasional  travel to multiple sites and community meetings

Physical Demands:

Ability to transport materials, routinely moves equipment or supplies weighing up to 30 lbs.
