Be part of the solution!  Health Federation of Philadelphia, in partnership with the School District of Philadelphia, is seeking a dynamic, experienced, passionate Pediatric or Family Nurse Practitioner looking to make a difference by serving in a new role as a clinical care coordinator across a group of public elementary schools in North Philadelphia.  The Clinical Care Coordinator is an advanced practice nurse (nurse practitioner) who works autonomously or in collaboration with other healthcare professionals to deliver quality care.  The clinical care coordinator will offer a wide range of healthcare services to students and their families - many confronting challenging health and social issues including the promotion of a healthy lifestyle to disease prevention, management of chronic health conditions, identification of an acute health need and health counseling.  The clinical care coordinator sees the student and parent(s)/guardian as partners in their care with an emphasis on health and well-being to maximize the student’s ability to learn and thrive.


Clinical Practice

  • Develops and monitors implementation of effective clinical practices in the delivery of student health support services
  • Develops care plans and implements treatment plans based on clinical decisions
  • In collaboration with the school nurse, identifies the health status of students through a comprehensive health history, recommending appropriate diagnostic and screening tests as indicated
  • Provides one-time prescriptions of medications as indicated and communicates with the primary care provider regarding current health status and need for follow up care
  • Provides excellence in school health services in accord with established standards of care and practice by the Pennsylvania Board of Nursing as well as the credentialing bodies for nurse practitioners and other state, local and federal regulations
  • Participates in mandated service delivery, including the Commonwealth of PA screenings and examinations and the Philadelphia Department of Health immunization compliance regulations
  • Serves as a resource to the school nurse; works collaboratively to manage complex health needs of students.  Supports the school nurse in public health outreach to families to ensure school readiness for students.  Serves as a primary clinical consultant to school nurse

Community Health Services

  • Coordinates care with community resources and establishes a system to link students, parents, guardians and primary care providers
  • Establishes and maintains positive relationships with managed care companies as well as other public, private and governmental organizations.  Serves as a liaison with stakeholders invested in the health care needs of children
  • Participates in school sponsored community health education and health promotion initiatives
  • Ensures adherence with all state, federal and local mandated guidelines governing activities related to school-based health services
  • Cooperates with community health organizations, in coordination with the school nurse, in ensuring that immunizations for students are current.  Assists in immunizations as needed
  • Serves as a consultant to school and central office staff and works collaboratively on student-related health needs and the planning, development and implementation of school-based health services
  • Supports the implementation and maintenance of programs and practices that support the goals and objectives of the School Board for healthy children and families
  • Identifies and participates in the development of grant proposal opportunities

Health Service Evaluation

  • Identifies best practices for delivery of health services across the District and  
    within non-public schools as requested
  • Utilizes the Student Information System (SIS) and other data sources to evaluate the effectiveness of school health services
  • Reviews the School Nurse Procedures Manual and references it as a basis of support for schools and school nurses


  • Master’s degree in nursing from an accredited university
  • PA Board certification as a nurse practitioner (CRNP)

Essential Skills/Experience

  • Minimum of 3 years of experience as a NP in pediatrics or family practice preferred
  • Current registered nursing license (RN) in PA in good standing
  • Excellent written and verbal communication skills
  • Awareness of standards of practice, NCQA guidelines and Medicaid contracts and benefit systems preferred
  • Eligibility for prescriptive authority through the Board of Nursing or Medical Board in PA required
  • Responsible for remaining current in her/his practice and professional responsibilities, to include but not be limited to chronic disease management and acute medical or mental health needs
  • Ability to work collaboratively with school nurses, other NP’s and physicians as well as other members of the health care team
  • Ability to demonstrate empathy,  understanding, and a trauma-informed and resiliency approach to care
  • Ability to establish and maintain good working relationships across disciplines and groups utilizing an understanding of integrated care of the whole child
  • Ability to demonstrate mutual respect and build a trusting, trauma-informed relationship with students, families and team members
  • Ability to make meaningful contributions to services, programs and practices and to share and receive information
  • Ability to work towards common goal of healthy students and families
  • Builds on the strengths of the student and family to promote health and well being

Work Environment:

Public elementary school setting in North Philadelphia

Position Type and Work Schedule:

Full time position, typical hours are Monday through Friday from  8:30 am to 5:00 pm.  Some evenings and weekend hours are likely.  Flex office schedule options available with supervisor approval. 


Occasional travel to multiple sites and community meetings

Physical Demands:

Ability to transport materials, routinely moves equipment or supplies weighing up to 30 lbs.
